Programs that maintain information about community resources that are appropriate for a specific target group or human services sector (for example, youth programs or addiction services) and which link individuals who are in need of specialized services with appropriate resources and/or which provide information about community agencies and organizations that offer specialized services.

Community Living Hamilton Child Care Supports Location

Provides Special Needs Resourcing supports that help childcare programs include children with special needs.  Staff work directly with the childcare centre to support the inclusion of your child in the classroom.  Staff also assists with information and referral as well as the transition to schoo

This program helps people aged 16 and older with a developmental disability who are looking for adult developmental services in the following ways:

Offers supportive programming for women from Hamilton's diverse racial and cultural communities, in particular women who are immigrant, newcomers, and refugees.

Provides assistance for individuals with developmental disabilities and their families to help them navigate the developmental services system in Ontario.

A non-profit community mental health agency which provides services to adults with serious mental illness.  Promotes mental health through outreach, social action, advocacy, education and research.

The program assists adults with developmental disabilities living with their families or in the community with little or no support by:

Culturally safe and centralized call centre that connects Indigenous residents in Hamilton to local and government resources relevant to their identified needs. Navigators will support families in completing applications and create warm transfers with service providers.
