Provides coordination and case management of community resources, advocacy, emotional support, and guidance to adult individuals with a developmental disability.
The Adult Protective Services Worker program (APSW) provides support to those living in the community on their own with little or not significant support.
The Adult Family Support Worker program (AFSW) provides support to those living at home with their family.
Services include:
- identification of a person's needs, strengths, resources, and goals
- Identification of/and liaison with additional required services
- Individualized planning
- service coordination
- case management and advocacy
- emotional support and guidance
- monitoring and follow up
