Provides health-related items to people receiving Ontario Works or Ontario Disability Support Program (ODSP) funding and people on low income.

Benefits covered include:

Adult day programs user fee

Affordable Transit Pass

Bathroom assistive devices such as grab bars and commodes

Custom foot orthotics and orthopedic footwear

Dental and Dentures services

Diabetic supplies

Eye glasses

Hearing aids and batteries

Hospital beds and mattresses

Medical taxi transportation

Personal emergency response

Surgical, incontinence, and ostomy supplies

Wheelchair batteries and repairs


Age 18 years and older; Ontario Works and Ontario Disability Support Program recipients * people on a low income who are 18 years or older and a resident of Hamilton * program does not fund equipment, devices, or services that have already been ordered or purchased


For Ontario Works (OW) and Ontario Disability Support Program (ODSP) recipients, discuss needs with existing case manager (OW: 905-546-4800, ODSP: 905-521-7280). For applications who are not OW or ODSP recipients, an application form is completed to determine individual or family eligibility. Mail to Ontario Works East End Office or drop the form off at East End, Central, Mountain, or CDC office. A friend, family member, or advocate may assist in completing the application process. Applicants will be asked about: income including CPP, OAS, pension, and other earnings; assets including cash, bank accounts, RRSPs, stocks, bonds, and secondary property; living expenses including rent/mortgage, utilities; and the cost of the special support item.
