Eligibility
Children and youth must be between 3 and 18 years of age, legal residents of Ontario with a valid Ontario Health Card, and registered with Easter Seals Ontario.

Application Instructions
Read, complete, and submit an Equipment Funding Application (found on the website) for each piece of equipment required. Children must be registered with Easter Seals Ontario.
