Psychiatric Rehabilitation

Programs that integrate treatment strategies associated with psychiatry, occupational therapy and psychiatric case management to help severely mentally ill individuals stabilize their clinical status, increase their ability to function within their environment with as little ongoing professional intervention as possible, and improve their overall quality of life. Services are provided within an intensive case management system and may include medication and supportive psychotherapy to alleviate symptoms; training in interpersonal and independent living skills with a focus on personal care and management, leisure skills, social interaction skills and vocational preferences and aptitudes; cognitive retraining; family psychoeducation; peer support; and assistance in gaining access to the resources and benefits to which they are entitled. The individual is actively involved in developing the treatment plan and selecting the types of training that will be relevant to his or her ongoing roles and relationships.

 

Services

A community-based program offering counselling and case management services to individuals with serious mental illness. Social work staff assess mental status, needs, and abilities to assist individuals in developing a plan and setting realistic goals.

Intensive Case Management Services engage individuals with a serious mental health concern who currently live in a home with a fixed address.  City of Hamilton Public Health nurses work with residents to increase independence and participation in their community, as well as address health concern

Street Outreach Services that engage individuals who are experiencing mental illnesses and are homeless or living in shelters. City of Hamilton Mental Health and Street Outreach staff work in the community at many locations.

Offers care for mental health and addiction issues to young people age 17 to 25 in a safe environment.  Clinical mental health care is offered in an early intervention stream, a transition support stream, and from a mobile team.  Drop-in counselling is also available.  The Centre has staff who ar
