Centre responds to the health needs of individual patients and their families in Hamilton and across the Hamilton Niagara Haldimand Brant Local Health Integration Network, by providing accessible services, and empowering individuals with wellness and illness prevention information.

Charitable organization providing social supports for children, families, adults, and seniors. Focuses on reducing poverty and social isolation. 

Health centre focused on family and community health. Health professionals assess and treat non-life threatening injuries or illnesses and provide referrals to other local health services and personal development groups. Community-led health teams develop programs to improve the social supports and conditions that affect long-term health.

Hamilton Community Garden Network (HCGN) provides resources and tools needed to start, run, or participate in community garden development.

HCGN is currently organized by the Neighbour to Neighbour Centre's Hamilton Community Food Centre.

Provides primary care to low-risk pregnant women throughout their pregnancy, labour, and birth and during first six weeks following birth. Family-centred prenatal and postnatal care for home or hospital births, from practitioners with privileges at McMaster and St. Joseph's hospitals. Also offers clinical placements for midwifery students.

Provides a range of support services and resources for the Indigenous community of Hamilton. 

A multi-use community centre houses a number of creative projects, and provides both short-term and long-term space for community meetings, workshops, events, and artistic/commercial purposes. 

A non-profit agency supporting children, youth, adults, and families in multiple locations throughout Brant and Haldimand counties through the delivery of a range of health and social programs.
