Harm reduction and managed alcohol program for women and men experiencing homelessness, addiction, and serious health problems. Up-front discharge planning for effective transition to the community.
A program to divert youth from the shelter system and homelessness, the Youth Diversion Program provides:
Services designed to help newcomer, immigrant, and refugee women address their unique needs through community resources and referrals, short term sessional counseling, and workshops.
Supports and services offered:
Free weekly groups for pregnant individuals. Discussions with a Registered Nurse and Registered Dietitian include pregnancy, labour and birth, managing stress, eating well and cooking healthy food, breastfeeding, and community supports.
In-home health care offered in private homes, long-term care homes, group homes, hospitals, and community settings.
Services include:
Specialized service and treatment for individuals who have behaviours related to a diagnosis of dementia. Program provides service to Local Health Integrated Network (LHIN) 4 (the regions of Niagara, Brant, and Halton).
