HOMES Program

Ministry of Health funded program for individuals living with mental health issues who require housing & support in the Hamilton community. Support is provided seven days a week utilizing various models and approaches to housing with supports. It has both on-site teams and mobile teams in the recovery based support delivery. Housing choices range from bed/sitting rooms to detached homes.

Eligibility

Age 16 years and older; Individuals living with mental health issues, are homeless or at risk of being homeless and require supportive housing.

Application

Initial phone call to Director, will screen for eligibility (re: homelessness/mental illness) * When units are available, person referred will be seen for Intake interview and then matched with appropriate housing.
