    Eligibility

    Age 17 years and younger

    Application

    Referral through CONTACT Hamilton, except the Outreach Team (referral via physician). On your visit bring: your child's Ontario Health Insurance Card (OHIP Card), a list of all past and current medications including all vitamins, supplements, and herbal remedies, and forms or information you/your child have been asked to complete, any other forms related to your child's care such as reports from previous assessments, letters, and school information.
