Provides 24-hour residential hospice and palliative care to individuals with a life-limiting illness. Offers person-centered care that promotes comfort, pain and symptom management, dignity, and quality of life, all while respecting an individual's culture, beliefs, and values. Care is provided by a team of nurses, allied health professionals (social work, grief counsellors, spiritual care), support staff, and trained volunteers.

Eligibility
Applicant must:
- Be 18 years or older
- Be in final stages of life, with a prognosis of 3 months or less and/or a Palliative Performance Scale (PPS) score of 40% or less
- Have a need for nursing and PSW care, with a focus on pain and symptom management, in addition to other social and emotional support
- Have a Do Not Resuscitate (DNR) agreed to, prior to admission
