Centre de Santé Communautaire Diabetes Education Program

Offers education and support to clients by informing, educating, and equipping people with diabetes to better manage their disease and reduce the risk of diabetes-related complications, as well as reducing the risk of developing diabetes for people who are pre-diabetic or are at risk of developing diabetes.

Eligibility

All francophone people age 18 years and older with diabetes, pre-diabetes, or at risk of diabetes in the Hamilton and Niagara regions are eligible to participate in the Diabetes Education Program, whether or not they are clients of the Centre de Santé Communautaire medical team. Referrals can be made by family physicians or nurse practitioners.
