Eligibility
To be eligible for this service, the person referred needs to be:
- Living in Hamilton
- 16 years of age or older
- Diagnosed with a serious and persistent mental illness or concurrent disorder
- Under the care of a physician (some services require a Hamilton physician)
- Willing and able to participate in working on goals, and requires to support to maintain wellness in the community
