Behavioral Supports Ontario Community Outreach and Support Services

Provides information and access to assistance for older adults with age related cognitive impairments and responsive behaviours that impact their daily functioning and/or the ability to maintain relationships.

Team members are trained to identify client needs and directly link them to the appropriate services.

Outreach offers services such as:

Eligibility

Older adults with cognitive impairments due to mental health problems, addictions, dementia or other neurological conditions who exhibit or are at risk of exhibiting responsive or challenging behaviours such as aggression, wandering, physical resistance and agitation.
