Case Management Program

Provides assistance for individuals with developmental disabilities and their families to help them navigate the developmental services system in Ontario.  Case Management is a collaborative, client driven process for the provision of quality health and support services through effective and efficient use of resources.  Case Management supports the client's achievement of safe, realistic, reasonable goals within a complex health, social and financial environment.  

CHOICES supports the National Case Management Network definition and principles of Case Management.  This assistance is focused on the client's development, in combination with the available resources in the developmental services system as funded by the Ministry of Community and Social Services.
