Provincial Ontario program that pairs Ontario residents with local doctor or nurse practitioners that are accepting new patients.

Offers primary health care services to people living in Hamilton who face barriers to traditional health programs. Doctors and nurses assess and treat minor illnesses and injuries.

Staff inlcudes:

  • family physicians and physician assistants
  • nurse practitioners and registered nurses
  • pharmacist consultants
  • respiratory educators

Provincial Ontario ministry responsible for:

  • providing fast access to health care
  • delivering coordinated care in communities
  • educating patients to make the right decisions about their health
  • sustaining the health care system

Comprehensive foot care program that focuses on the assessment and treatment of feet and the prevention of further diabetic foot complications like ulcers and amputations in people living with diabetes. All foot care is done by a team of registered foot specialists and registered practical nurses with advanced foot care training.

Funding for the program is provided by the Hamilton Niagara Haldimand Brant LHIN.

An on-site kiosk that allows individuals who do not have a permanent address to acquire an Ontario health card. A temporary health card number enables participants to access health care services and program staff assist in attaining a permanent card. Health cards are provided by the Ontario Ministry of Health and Long-Term Care.

Accredited long-term care homes offering nursing and personal care on a 24 hour basis. Rooms available for both long-term and short-term/respite stays, please contact for exact numbers available. Additional services include: laundry, housekeeping, personal hygiene and medical supplies, meals, pastoral services, and social and recreational programs.  

Promotes oral health among individuals that are under-served and lack access to adequate oral health care. Services include cleaning, fluoridation, pits and fissure sealants, and check-ups. 

A medical group of five hospitals, a cancer centre, and an urgent care centre offering acute and specialized health services. Each site has specific areas of expertise. An academic environment created by an affiliation with the McMaster University Faculty of Health Sciences offers patients the benefits of innovative treatments.

Provides comprehensive outpatient and inpatient respiratory care as the regional respiratory service for the City of Hamilton and the Hamilton Niagara Haldimand Brant Local Health Integrated Network (LHIN). Chest program encompasses respiratory medicine and affiliated head-and-neck and thoracic surgery services, all located on-site.
