A community-based program offering counselling and case management services to individuals with serious mental illness or concurrent disorders. Social work staff assess mental status, needs, and abilities to assist individuals in developing a plan and setting realistic goals. Skills training, referrals to community resources, and advocacy are provided as required. Provides long-term intensive case management to individuals diagnosed with a combined or co-occurring substance use disorder and/or gambling disorder and serious mental illness.
Goals of the program are to:
- Reduce the likelihood of re-hospitalization
- Use appropriate community resources for client's maximum benefit
- Develop greater independence in areas of self-care, life skills, community and professional involvement

Individuals with a diagnosis of a serious mental illness/concurrent disorder from their family physician or psychiatrist who live in the City of Hamilton
Referrals accepted from medical professionals, social service agencies, family members, or from the individual. Participation is voluntary. All referrals are processed through IntAc.
